- The Department of safety website may listen to calls or chat following a complaint.

- The Department of security shall be entitled to suspend or cancel the registration of a patient or therapist.

Appendix No. 1: Patient Identification Prior to Treatment
I am aware that my call is fully documented by the remote health service, and by marking the box stating "I accept the terms of use", I have consented thereto and hereby declare that the personal information and identification data I provide, on behalf of myself or another individual benefitting from the service, are full and accurate.
Appendix No. 2: Notifying Patient and Obtaining Her/His Consent
I am aware of the following:
- By approaching this remote health service, I hereby give my continuous consent as required by law for treatment, including professional consultation and transferring the medical information to other caregivers, as customary in an organization providing health services and in correspondence to medical and professional considerations.
- The service proposed as a remote health service involves video consultation calls, and management of a medical file documenting the consultation.
- In some cases, I will be referred to another medical encounter, in order to complete the necessary medical examination.
- In any case of an urgent problem, an emergency or a physical and/or mental distress, I may approach for immediate medical care at any emergency medicine center of a hospital or any other medical call center, providing emergency medical services, such as "Magen David Adom" (Red Shield), "Red Cross", or "Red Crescent".
- The information I provide will be documented in the service's medical file, according to the law.
- The service's caregivers will do their utmost to maintain individual privacy and medical secrecy.
- There are limitations to maintaining privacy and medical confidentiality; also, communication failure and sudden disconnection from service may occur. The management does its utmost to secure the information I provide. It is my exclusive responsibility to ensure my own privacy in terms of the personal medical information appearing on the personal digital screen from which the service call is placed (for example; computer screen, terminus, telephone, etc.) which may be exposed to individuals near me.
- Contacting the service through insecure communication may be exposed on the Internet. The supplier will do his utmost to ensure the privacy of information in his records, but may not ensure privacy of information throughout the communication from the endpoint from which I use the service, and the information saved at my endpoint. Hence, it is recommended, after each use, to remove the information and/or identification data, including username, password and/or any other item of data from the endpoint I have used, so as to narrow down the prospects of access to information related to me and/or the service, by any individual who has not been given the same access. Securing the information on my endpoint is my exclusive responsibility. The service provider is unable to secure that information.
- Contacting the service through social media may result in sharing medical information with others.
- The service's charge is $5 each month for folder maintenance.
- The service's charge is 14% above the doctor's/therapist's price. The percentage is subject to changes once per period as decided by the management.
- Tax Invoices will be sent by mail.
